RISK FACTORS - I acknowledge that the use of the facilities, equipment, and services provided by The Wellness Studio in Geelong (Floatation, Infrared Sauna, Salt Therapy, & ancillary activities like walking on wet bathroom floor, showering etc.) involves risks such as risk of property damage, bodily injury and possible death.
POLICIES AND PROCEDURES - I understand all of the policies and procedures disclosed to me in relation to the activities, facilities and/or equipment, and recognise that the safe and proper use of the facilities, equipment or participation in the activity is dependent upon careful following of said policies/procedures.
PREREQUISITE PHYSICAL AND MENTAL HEALTH - I acknowledge that I have the requisite level of physical and mental health for proper and safe use of the equipment and facilities. I am not under the influence of any drugs/alcohol, either recreational or prescription, which would cause panic, claustrophobia or otherwise interfere with the proper and safe use of the equipment and facilities. I agree that any questions in relation to what skills or training is necessary for proper use of the equipment, facility, or the activity itself will be directed to the appropriate staff member on site. I must disclose any illness or condition that may put me at risk, whereby The Wellness Studio in Geelong has the right to refuse my participation until written medical clearance is provided. I confirm that the information provided regarding my medical history is true and correct. If any aspect of my medical condition has changed since my last visit, I understand that it is my responsibility to advise staff of relevant changes.
PAY - I agree to pay for any damages to property or indemnities that I, or any minors that I sign for, cause negligently, wilfully, or otherwise.
CANCELLATION AND LATE POLICY - A cancellation fee will be charged if you fail to provide notice of cancellation on recurring occasions. Arriving late may require us to shorten the length of your session, with full charges applied, so as not to inconvenience other customers.
LOST PASS OR VOUCHER - Our vouchers do not have an expiry; however if you lose your voucher and there is no proof of purchase, it is no longer redeemable.
